FORMS 2019-2020 Season
In order for you to be eligible to practice and play on game day you must have submitted the required CMS forms with payment and Registered with CJRA.

- Sign up through Charlotte Junior Rugby Association (CJRA) – Register at CJRA – Select “Providence High School Boys Rugby” Cost $125

- Fill out all CMS paperwork at https://cmsathleticzone.sportngin.com/register/form/259200001

- If you prefer completing paper forms please print and hand in at practice with you Physical and a check made payable to “Providence Rugby Club” for $75 – Print Forms

- Please forward the “Providence 2019-2020 Ticket to Play Registration Receipt” email to plumwhitney@gmail.com so that we can track who has completed their paperwork.

- Pay the $75 School Fee via the online school payments portal Payment Link
- Student physical good through end of season must be handed in at Practice.

You do not need to CIIP your player this year, this will be done by CJRA when you register.

Each Player is expected to participate in the club fundraising activities (VIC, car wash cards etc.)This is also a requirement for “Lettering”
In Lieu of participation, a donation of $100 to the club will be accepted and can be made here – Payment Link
Players will have the opportunity to purchase other PHS Rugby gear, including a rain jacket, duffle bag
and other clothing. Ordering information will be provided prior to the start of the season.
Parents and families are encouraged to show their support by purchasing and wearing PHS Rugby
gear and volunteering throughout the year. Ordering information will be provided prior to the start of the
season.
